Information for Seniors:

MCV4 – required for colleges

Men B – a 2 shot series that is the optional 5th strain of meningitis

6th grade – Mcv4, Tdap required; HPV optional

Deadline to sign up is April 28, 2022

*If you plan to pay out of pocket, please submit your info!  You will put “NA” in all insurance blanks.
